Walk down the supplement aisle at any pharmacy and it's easy to feel like you're missing something. Multivitamins, fish oil, calcium, magnesium, protein powder — shelf after shelf, each one promising more energy, stronger bones, or a sharper mind. If you've ever stood there wondering what you actually need, you're not alone.
By some estimates, 7 in 10 older adults take at least one supplement, and nearly a third take four or more. Most people start because they want to feel healthier, protect their bones, or support their heart — not because a doctor told them to. And that's exactly where things can get complicated.

Prescription medications go through years of clinical trials and have to prove both safety and effectiveness before the FDA allows them on the market. Dietary supplements don't have to clear that bar. Under current law, a manufacturer can sell a new supplement without ever proving it works — the FDA only steps in after a product is already on shelves, and only if it's shown to be unsafe.
At the same time, the supplement and wellness industry is a multi-billion dollar market, often with a marketing budget that outpaces the actual science behind the product. A confident label, a celebrity endorsement, or a glowing social media testimonial doesn't require the same evidence a prescription drug does — it just requires a good ad. That gap between how a product is marketed and how thoroughly it's been studied is exactly why quality can vary so much from bottle to bottle, and why looking for third-party verification (USP or NSF) matters — those seals exist precisely because the government doesn't require that scrutiny before a product reaches you.
If you're taking three or more prescription medications — which is common as we age — adding a supplement isn't as simple as it sounds. Many supplements are processed by the same liver pathways that break down prescription drugs, meaning they can speed up, slow down, or intensify how a medication works in your body.
Some of the clearest examples are supplements that share an effect with a medication you're already taking: garlic, ginkgo, ginseng, and high-dose vitamin E or fish oil can all add to the effect of a blood thinner, increasing bleeding risk. Others can compound the effect of blood pressure or blood sugar medications more than intended.
What makes this harder to predict than a typical drug-drug interaction is that supplements aren't required to be tested for interactions before they reach the shelf — so for many products, no one actually knows the full risk. That uncertainty is exactly why it's worth a conversation before adding anything new, especially if you're already managing a chronic condition.
The one thing worth remembering: something that worked for a friend, a family member, or someone in a glowing online review may not do the same thing for you. Your medications, your kidney and liver function, your diet, and your specific health conditions all change how a supplement behaves in your body. What helps one person can be useless — or even risky — for someone else.
Supplements aren't inherently good or bad — they're tools, and like any tool, the right choice depends on the job in front of you. What matters is making that choice based on your own health picture, not a headline or a friend's experience.
