A primary care physician is the doctor you see for most of your everyday health needs, including annual physicals, preventive screenings, sick visits, and management of ongoing conditions such as diabetes and high blood pressure. Having a consistent PCP means you have a provider who knows your medical history, can monitor changes in your health over time, and can coordinate your care when specialists are needed.
Please bring a photo ID, your insurance card, a current list of your medications and dosages, and any recent test results or medical records from previous providers. If another provider referred you to our office, please also bring any referral paperwork or relevant records.
Your PCP is generally appropriate for non-emergency concerns such as ongoing symptoms, medication management, follow-up visits, chronic conditions, and preventive care. Urgent care may be appropriate for problems that need prompt attention but are not life-threatening, such as minor injuries or infections. For a medical emergency, including symptoms such as chest pain, severe difficulty breathing, severe bleeding, or other potentially life-threatening symptoms, call 911 or seek emergency medical care.
Yes. One of the advantages of primary care is having a provider who looks at your overall health rather than treating each condition in isolation. Your PCP can help manage conditions such as diabetes, high blood pressure, and high cholesterol together and coordinate specialty care when necessary.
We do our best to see patients promptly and may have same-week availability for more immediate concerns. Annual physicals and routine follow-up appointments can be scheduled online or by contacting our office.
Appointments must be cancelled or rescheduled at least 24 hours before your scheduled appointment. Patients who do not attend their appointment or who cancel or reschedule without providing at least 24 hours' notice will be charged a $25 No-Show/Late Cancellation Fee.
We accept most major insurance plans, including Medicare. An annual physical focuses primarily on preventive care and wellness. Your visit may include a review of your medical history, a physical examination, preventive screenings, and recommendations based on your age, medical history, and individual health risks.
Most insurance plans require at least 12 months between covered annual physical examinations. Coverage varies by plan, however, and your insurance company ultimately determines your eligibility and benefits.
If your insurance company determines that you are not yet eligible for another covered annual physical, you may be responsible for charges not covered by your plan.
Yes. However, an annual physical is a preventive visit. If you ask your provider to evaluate or treat a new or ongoing medical condition during the same appointment, those services may be billed separately according to your insurance benefits.

Yes, you can send a message through the patient portal for refills, or you can ask your pharmacy to request refills on your behalf (usually faster in most cases).
Some insurance companies require additional information from your healthcare provider before they will cover certain medications. This process is called a prior authorization.
Medication prior authorization requests may take 3–5 business days to process after our office receives the request from your pharmacy or insurance company. Additional processing time may be required by your insurance company after our office submits the requested information.
Yes. We can complete appropriate medical forms when we have the medical information and documentation necessary to do so. We do not fill out these forms for first-time patients — they are reserved for established patients in the practice.
Yes. All medical forms are subject to a $50 form completion fee.
Possibly. Depending on the type of form and the medical evaluation required, an office visit may be necessary before your provider can complete the form.
FMLA, disability, and other medical forms are generally completed within 5 business days after all required information and documentation have been received by our office. Please make sure all required sections and supporting documentation are provided to avoid unnecessary delays.
Yes. Our office can complete applicable immigration medical forms and related paperwork when the required documentation has been provided.

No. Mobee Medical Associates does not prescribe or manage long-term (monthly) opioid pain medications. Our practice is committed to providing safe, evidence-based medical care. We may offer short-term opioid medications based on our evaluation (after surgery, urgent/emergent situations, etc.) at our discretion. Long-term use will warrant a referral to a pain management specialist of your choice.
No. We do not prescribe or manage long-term benzodiazepine medications such as Xanax® (alprazolam), Ativan® (lorazepam), or similar controlled substances. You will be referred to an anxiety specialist (psychiatrist) of your choice.
If medically appropriate based on your provider's evaluation and screening, a provider may prescribe a one-time, short-term medication. A prescription is not guaranteed and will depend on the provider's medical evaluation.
If ongoing treatment is medically necessary, you may be referred to a pain management specialist, psychiatrist, or another appropriate specialist of your choice for continued care.
This depends on your medical needs and insurance requirements. If your PCP determines that specialty evaluation is appropriate, we can help coordinate your referral and provide relevant medical information to the specialist. Some insurance plans also require a formal PCP referral before covering specialty services, so patients should verify their individual insurance requirements.
Many follow-up appointments, medication check-ins, and other appropriate concerns may be handled through a telemedicine visit. Please contact our office to determine whether your specific medical concern is appropriate for a virtual appointment.
Established patients can use the patient portal for convenient communication with our office, including requesting medication refills and sending appropriate messages to the care team.
