Mobee Medical Associates ("we," "us," or "our practice") is required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of this notice while it is in effect.

Our Commitment to Your Privacy

We may contact you to provide appointment reminders or information about treatment alternatives or other health-related benefits and services that may be of interest to you.

Other Permitted or Required Uses and Disclosures

We may also use or disclose your PHI without your written authorization for the following purposes, as permitted or required by law:

As required by law, court order, or subpoena
For public health activities, such as reporting disease outbreaks or adverse events
To report suspected abuse, neglect, or domestic violence to appropriate authorities
For health oversight activities, such as audits or investigations
For judicial and administrative proceedings
For law enforcement purposes, under limited circumstances
To coroners, medical examiners, or funeral directors as necessary
For organ, eye, or tissue donation purposes

Uses and Disclosures Requiring Your Written Authorization

Other than as described above, we will not use or disclose your PHI without your written authorization, including for most marketing purposes and any sale of your PHI. You may revoke an authorization at any time in writing, except to the extent we have already acted in reliance on it.

Your Rights Regarding Your Health Information

You have the following rights with respect to your PHI:

Right to Inspect and Copy. You may request to inspect and receive a copy of your health record.
Right to Amend. You may request that we amend your health information if you believe it is incorrect or incomplete.
Right to an Accounting of Disclosures. You may request a list of certain disclosures we have made of your PHI.
Right to Request Restrictions. You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requested restrictions.
Right to Request Confidential Communications. You may request that we communicate with you about your health information in a specific way or at a specific location.
Right to a Paper Copy of This Notice. You may request a paper copy of this notice at any time, even if you have agreed to receive it electronically.
Right to be Notified of a Breach. You have the right to be notified if a breach of your unsecured PHI occurs.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current notice will be available at our office and on this website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our practice or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
